Health Insurance Portability and Accountability Act (“HIPAA”) Privacy Information

To Patients and Interested Parties:

Your privacy and the protection of your personal health information have always been of great importance to everyone who works at Critical Care Systems.  Our mission is to provide high quality healthcare and build relationships with our customers based on trust.  This cornerstone of trust is built on our firm commitment to respecting the privacy and confidentiality of your health information.

On April 14, 2003, the Federal Office of Civil Rights began enforcing new rules regarding the use and disclosure of patient health information between patients, providers and health plans.  As part of these new rules, Critical Care Systems and other healthcare providers are required to provide you with a copy of their Notice of Privacy Practices and to request your signature acknowledging receipt of this notice.

Critical Care Systems restricts the use of your personal health information in accordance with HIPAA and applicable state laws.  Critical Care Systems does not sell or distribute your personal health information to other companies for marketing purposes.  Critical Care Systems will only use and share your information in accordance with this Notice of Privacy Practices for your treatment, payment and healthcare operation needs and for other limited activities allowed by law, such as health oversight activities and government requests.
