Procedures, forms and deadlines

All residency candidate application materials must be received no later than the application deadline: January 6, 2012.

Please follow these instructions for application to one or more of the five Critical Care Systems PGY1 pharmacy residency programs.

Checklist for application

• Residency Application for Consideration (Return signed and completed per instructions.)

• Critical Care Systems Employment Application (Return signed and completed.)

• Applicant Letter of Intent (up to two typewritten pages)—Please explain why you are applying for a residency, the experiences you hope to gain, and the specific reasons why you are interested in pursuing a Specialty Infusion PGY1 Pharmacy Residency at Critical Care Systems. (Forward with application materials.)
  
  
• Curriculum Vitae, including employment, professional activities (Forward with completed application forms.)
  
  

Arrange for the following, which must be sent directly to Critical Care Systems no later than January 6, 2012:

• Official transcripts from all colleges attended
  
  
• Three professional letters of recommendation (per the instructions on CCS Residency Recommendation Request)

After receipt of completed applications, the Pharmacy Residency Committee will conduct telephone interviews in January and February and may request an onsite interview (upon invitation) at the Critical Care Systems Corporate Office (Nashua, NH) or at a residency program site location.
